Short Forms

A short form consent is used to enroll subjects with limited English proficiency (LEP) into a research study when a version of the consent form translated into the subject's language is not available. A short form is intended to allow the enrollment of LEP subjects when it was unexpected that such a language would be necessary.

To receive approval for the use of these short forms, please indicate in your study protocol that you will be enrolling subjects with LEP. Excluding subjects on the base of their lack of English proficiency should be justified in the protocol.

The procedure for using a short form is described in the IRB Policies and Procedures - Chapter 44 - pg.172 (PDF).

A step-by-step guide to using a short form is provided as a cover sheet on each document.

Please note that Emory has a strict policy about using family members and study staff as interpreters. For more information, access Family Members as Interpreters Guidance (DOCX).
